  Video and Photography Release Form
  In effort to communicate the fruits of our program to the greater Beatitudes community and to advertise to current and future families, we would like permission to use photographs and videos from our program for flyers, parish and diocesan publications, and our parish website. No first names will be published. If there are any concerns with pictures or videos posted, Beatitudes' Faith Formation can be contacted and the post(s) will be promptly removed.

I understand that the parish provides a place for my family to grow and be nourished in our personal relationship with Jesus through prayer, study of scripture, generosity and liturgy. I promise to support my child's faith formation at church and home by:

  *Family attending and participating in Sunday Mass.
  *Every week praying daily with family and on my own.
  *Attend all meetings for parents.
